Register as New Patient
To register with the practice, please bring proof of address in our catchment area (map opposite) and photo ID together with details of your previous address in the UK and of your General Practitioner (without this information we will be unable to register you.) I
If you don't have a medical card you will be given a registration form to fill in which will complete the registration process with the practice.
Registration forms and questionnaires are avalailable from reception or can be completed online via the links below. Please return both forms completed to the surgery.
It is important that we know about any relevant medical condition or history you may have. Therefore, if you are on regular medication or have an ongoing medical condition, please make an appointment to see one of the doctors as soon as possible.
Change of Personal Details
Please notify our reception staff of any change of name, address or telephone number in order to ensure that your medical records are correct.

Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
